What Is a Fat Pad Filler?

The ball of your foot and your heel sit on a built-in cushion of fat. It is there to absorb every step you take. When that cushion thins, the bones underneath lose their padding, and walking on a hard floor starts to feel like walking on stone. A fat pad filler restores cushioning where your own has worn away.

There are two ways to do it. We can move fat from elsewhere on your body and place it under the foot, or we can use a prepared cushion of donor fat tissue. Both aim at the same result: padding back where it belongs, so the bones are protected and each step lands softer.

Who It Helps

Fat pad thinning shows up as burning or aching under the ball of the foot, a feeling of walking on a pebble, pain that is worse on tile and concrete than on carpet, and calluses that keep returning in the same spot. It becomes more common with age, and it can follow years of high-impact activity, certain foot shapes that overload one area, repeated steroid injections, or previous foot surgery.

Patients who come to us for this have often already tried cushioned insoles from a pharmacy and found they flatten within weeks. Fat pad filler is considered when padding and orthotics alone are no longer enough to protect the area.

We start with conservative care, because for many patients it is enough. That means an examination and imaging to confirm the fat pad is genuinely thinned and to rule out the other causes of forefoot pain, then custom orthotics with padding built into the area that needs it, metatarsal support, and footwear guidance.

When too much of the natural cushion is gone for orthotics alone to protect the bones, a fat pad filler becomes the next step. It is performed by our board-certified foot and ankle surgeons at our boutique Manhattan offices, through a very small entry point, with the area numbed.

What the Visit Involves

The area is numbed, the filler is placed under the thinned part of the foot, and the entry point is small enough that stitches are often unnecessary. Most patients are walking out the same day in a protective shoe.

Your surgeon will tell you how long to stay in the protective shoe and when you can return to your normal footwear and activity, since that depends on where the filler was placed and what you do on your feet.

Recovery and How Long It Lasts

Expect some swelling and tenderness in the first days. Most patients notice the change in how the ground feels underfoot within the first few weeks, as the swelling settles and the new padding takes over the work.

Fat pad filler is a way of buying back cushioning rather than a one-time fix. How long it lasts varies with the material used, where it was placed, your weight and activity, and what you wear on your feet. We plan for the long term by pairing it with custom orthotics, so the restored padding is supported rather than left to carry the load alone.
