Tendons are the ends of muscles that attach to bones. Tendonitis means there is inflammation (or irritation) of the tendon. It can be caused by overuse, an injury, or due to an activity your body isn’t used to. The Achilles tendon is the tendon that at the back of the ankle and heel and is one of the most common tendons in the lower leg to become irritated.
As soon as you notice pain, remember RICE: Rest, Ice, Compress, and Elevate. If pain hasn’t improved after 2β3 days, visit a podiatrist. Stop the activity causing pain immediately. Avoid putting weight on the affected foot and ankle for 2β3 days to allow the initial inflammation to settle.
Apply an ice pack wrapped in a cloth to the back of your ankle for 15β20 minutes every 2β3 hours. Never apply ice directly to skin.
Wrap your ankle with a compression bandage to reduce swelling. Keep it snug but not too tight β loosen immediately if you feel numbness or tingling.
Raise your foot above heart level as often as possible, especially when resting or sleeping. This drains excess fluid and reduces swelling faster.
If the pain has not gotten better after 2-3 days, it is time to visit a podiatrist. Your doctor may place heel lifts inside of your sneakers to take away the tension on your Achilles tendon. Depending on the severity of the condition, you may be prescribed a brace or other device to help rest your foot/ankle and limit excess movement. You may also need physical therapy for 4-6 weeks to help with pain and swelling and to strengthen the tendon after it has healed. For more long-term support, custom orthotics (shoe inserts) can help prevent the problem from coming back.

Sometimes scar tissue builds up in a tendon and causes the tendon to lose its elasticity. At that point, chronic Achilles tendinitis can be treated by removing the scar tissue using a minimally invasive procedure like Tenex. This uses a wand that transmits ultrasonic energy to break through the scar tissue leaving healthy tissue behind. Downtime is minimal and return to normal activity is about a week.
Grab a resistance band (or even a stretchy t-shirt)! Place the band or shirt around the ball of your foot. Pull the two ends of the band or shirt towards you until you feel a nice stretch in the back of your leg. Hold this stretch for 30 seconds, and relax for 10 seconds. Repeat this 5-10 times for both legs!
Achilles tendonitis rarely requires surgery. However, if treatment is delayed or neglected, there could be further damage such as a tendon tear. This would require surgical repair, and although we can do this with minimally invasive techniques, it is better to prevent damage when possible. Don’t ignore your pain.
