Nail salons across New York City vary enormously in their sterilization and hygiene practices and when instruments are not properly autoclaved or disinfected between clients, the results can range from a minor nail fold irritation to a serious bacterial or fungal infection that requires weeks of treatment. Infections acquired during pedicures are a well-documented public health concern in New York State, yet many patients either don’t connect their symptoms to the salon visit or assume the infection will resolve on its own. A foot infection from a NYC pedicure most commonly results from non-sterile instruments penetrating the cuticle, nicking the skin during callus removal, or traumatizing the nail fold during aggressive cutting or pushing. The organisms introduced through these micro-injuries include Staphylococcus aureus, MRSA, Pseudomonas aeruginosa (notorious for green nail syndrome and causing infections in whirlpool foot baths), and most insidiously non-tuberculous mycobacteria (NTM) such as Mycobacterium fortuitum, which have been linked to cluster outbreaks at nail salons across the United States. NTM infections are particularly deceptive because symptoms may take three to six weeks to appear after the pedicure, presenting as slowly growing, non-healing nodules or ulcers that do not respond to standard antibiotics. At Healthy Feet NYC, our diagnostic workup for post-pedicure infections includes wound cultures on appropriate media, possible tissue biopsy, and targeted testing for atypical mycobacteria when the presentation warrants it.

Treatment for a pedicure-related foot infection depends entirely on the causative organism identified through cultures or biopsy. Standard bacterial infections caused by Staphylococcus or Pseudomonas typically respond well to targeted oral or topical antibiotic therapy selected based on sensitivity results. NTM infections, by contrast, require prolonged treatment often four to six months of multi-drug oral antibiotic regimens using agents such as clarithromycin, ciprofloxacin, or doxycycline, depending on the species and its sensitivities. Fungal nail infections (onychomycosis) that develop or worsen after traumatic pedicure procedures require prescription oral antifungals such as terbinafine or itraconazole, or combination topical therapy.
