A corn medically termed a heloma is a focal, conical thickening of skin with a dense central nucleus that develops in response to concentrated, repeated pressure, most commonly over bony prominences on the toes or in the interdigital web spaces. A callus, or tyloma, is a broader, flatter area of hyperkeratotic skin that forms on weight-bearing surfaces such as the plantar heel, ball of foot, and metatarsal heads as a protective response to diffuse mechanical load. Hard corns (heloma durum) form on dry, bony areas; soft corns (heloma molle) develop between toes where moisture prevents full keratinization. Both conditions result from the same fundamental cause chronic mechanical stress but require different treatment approaches based on lesion type, location, depth, and the bony or biomechanical abnormality driving their formation.

Conservative management, effective for the majority of patients, includes professional debridement for immediate relief, custom orthotics to redistribute plantar pressure, digital padding and aperture insoles to offload focal corn pressure, and footwear counseling. For persistent or recurrent lesions, in-office corn enucleation removes the nucleus precisely and comfortably. Surgical correction condylectomy, arthroplasty, or digital deformity repair is reserved for cases where bony prominence is the irreducible driver of recurrence, and is performed in accredited outpatient facilities with same-day discharge.
