Flat feet surgery is among the most technically demanding procedures in foot and ankle surgery because reconstructing the arch involves restoring the coordinated function of bones, joints, tendons, and ligaments simultaneously, and the surgical plan must be individualized to the specific deformity pattern each patient presents. At Healthy Feet Podiatry, our NYC flat feet surgeons do not apply a single operative template to every patient. We evaluate deformity stage, hindfoot flexibility, joint arthritic change, tendon integrity, and patient activity goals, then build a surgical plan designed to achieve durable correction with the least invasive approach the anatomy permits.

Flat feet surgery more precisely, flatfoot reconstruction or adult-acquired flatfoot deformity correction is indicated for patients in whom: conservative management including custom orthotics, physical therapy, and footwear modification has failed to control pain and functional limitation over a period of six or more months; deformity is progressive despite conservative care; posterior tibial tendon dysfunction has advanced to Stage II or III with flexible or semi-rigid hindfoot valgus; or the foot has developed rigid deformity or arthritis requiring joint-level correction. The surgical approach is determined by deformity stage: Stage II PTTD typically requires soft-tissue reconstruction (posterior tibial tendon debridement or transfer, spring ligament repair) combined with bony correction (medializing calcaneal osteotomy, lateral column lengthening, or Cotton osteotomy for forefoot varus); Stage III and IV deformity with arthritic change requires subtalar, talonavicular, or triple arthrodesis to correct rigid deformity and eliminate arthritic pain permanently.

Flat feet surgery at Healthy Feet Podiatry is performed in accredited outpatient surgical facilities, with individualized recovery protocols based on procedure type and patient factors. Soft-tissue procedures combined with calcaneal osteotomy the most common flatfoot reconstruction for Stage II PTTD typically involve a period of non-weight-bearing in a cast followed by progressive weight-bearing in a boot beginning at six to eight weeks, with return to regular footwear at four to six months. Arthrodesis procedures for rigid deformity involve longer protected weight-bearing timelines reflecting bone healing requirements, typically with return to activity at six to nine months. Minimally invasive techniques are used wherever the deformity pattern and required correction permit, reducing soft-tissue trauma and supporting faster rehabilitation.
