A regular bunion (hallux valgus) affects the first metatarsophalangeal joint at the base of the big toe — the most common forefoot deformity in adults. A tailor’s bunion, also called a bunionette, affects the fifth metatarsophalangeal joint at the base of the little toe, producing a similar bony prominence on the outer edge of the foot. The name “tailor’s bunion” derives from cross-legged sitting postures historically associated with the tailoring profession that placed chronic pressure on the fifth metatarsal head. While both conditions share the same fundamental mechanism — lateral deviation of the toe combined with medial flaring of the metatarsal head — they have distinct anatomical subtypes, different biomechanical drivers, and in the case of surgical correction, entirely different operative techniques.
