Gout in the big toe occurs when monosodium urate crystals formed from excess uric acid in the bloodstream precipitate into the first metatarsophalangeal (MTP) joint, triggering an acute inflammatory cascade. This joint is preferentially affected because of its relatively lower temperature compared to central joints, which lowers the solubility threshold for urate crystals and promotes their deposition. Attacks typically begin abruptly, often at night or in the early morning hours, and reach peak intensity within 12 to 24 hours. The joint becomes intensely swollen, hot, and red sometimes resembling a cellulitis or septic arthritis and even light touch causes pain disproportionate to any visible injury. Local measures including joint offloading, ice application, and temporary use of a stiff-soled shoe or surgical boot protect the inflamed first MTP joint during recovery. Once the flare resolves, we initiate urate-lowering therapy with allopurinol, titrated to achieve a sustained serum uric acid below 6 mg/dL, with prophylactic low-dose colchicine co-prescribed for the first three to six months to prevent mobilization flares during crystal dissolution.
