What Is Peroneal Tendonitis?

Peroneal Tendonitis is a condition involving inflammation or irritation of the peroneal tendons, which run along the outer side of the lower leg, ankle, and foot. These tendons help stabilize the ankle and support proper walking and balance.

This condition commonly develops due to overuse, repetitive motion, ankle injuries, sudden increases in physical activity, or biomechanical abnormalities such as high arches. Athletes, runners, dancers, and active individuals are particularly susceptible to Peroneal Tendonitis because of the repetitive strain placed on the tendons.

Without proper treatment, continued irritation can weaken the tendons over time and potentially lead to tendon tears, chronic ankle instability, and long-term pain.

Peroneal Tendonitis Treatment Options

At our New York foot clinic, we perform a detailed evaluation of your symptoms, foot structure, gait mechanics, activity level, and lifestyle to create a personalized treatment plan tailored to your needs.

Treatment focuses on reducing inflammation, allowing the tendons to heal properly, improving ankle stability, and preventing future injuries. Conservative treatment is highly effective in most cases and may include rest, ice therapy, braces, physical therapy, custom orthotics, and anti-inflammatory medications.

For athletes, performers, and patients seeking accelerated healing and recovery, regenerative therapies such as PRP and stem cell injections may also be recommended.

When diagnosed and treated early, Peroneal Tendonitis can often be successfully managed before significant tendon damage occurs. Most patients experience substantial pain relief, improved stability, and return to normal activity levels with conservative treatment.

Long-term support through orthotics, strengthening exercises, and proper footwear can help prevent future tendon irritation and ankle instability.

TREATMENT OPTIONS
RICE Therapy (Rest, Ice, Compression & Elevation)

Early treatment typically begins with rest, icing, compression, and elevation to reduce inflammation, swelling, and tendon irritation. Patients are encouraged to avoid activities that worsen symptoms.

NSAIDs & Anti-Inflammatory Treatment

Non-steroidal anti-inflammatory medications such as ibuprofen may help relieve pain and reduce swelling during the early healing phase.

Braces & Immobilization

Depending on the severity of the condition, supportive braces, walking boots, or other stabilizing devices may be recommended to reduce stress on the tendons and allow proper healing.

Physical Therapy & Rehabilitation

Physical therapy is commonly prescribed for approximately four to six weeks to strengthen the tendons, improve flexibility, restore ankle stability, and reduce the likelihood of recurrence.

Treatment Duration & Recovery

Recovery time depends on the severity of the tendon inflammation, activity level, and patient compliance with treatment. Mild cases may improve within several weeks, while more severe cases may require longer periods of rehabilitation and support.
Physical therapy programs typically last four to six weeks, although supportive orthotics and activity modifications may continue long term to prevent recurrence. Patients receiving PRP or regenerative therapies may experience improved healing and faster return to activity.

Frequently Asked Questions

Sometimes scar tissue builds up in a tendon and causes the tendon to lose its elasticity. At that point, chronic Achilles tendinitis can be treated by removing the scar tissue using a minimally invasive procedure like Tenex. This uses a wand that transmits ultrasonic energy to break through the scar tissue leaving healthy tissue behind. Downtime is minimal and return to normal activity is about a week.

Warm-up before and cool down after your workout routine. Slowly increase the intensity of your exercise over a period of time. If you’re using equipment at the gym, make sure you know how to use it safely and also with the form and posture. If unsure, always ask!

Grab a resistance band (or even a stretchy t-shirt)! Place the band or shirt around the ball of your foot. Pull the two ends of the band or shirt towards you until you feel a nice stretch in the back of your leg. Hold this stretch for 30 seconds, and relax for 10 seconds. Repeat this 5-10 times for both legs!

Achilles tendonitis rarely requires surgery. However, if treatment is delayed or neglected, there could be further damage such as a tendon tear. This would require surgical repair, and although we can do this with minimally invasive techniques, it is better to prevent damage when possible. Don’t ignore your pain. See your podiatrist as soon as possible.
