Chronic gout defined by frequent recurrent attacks, persistent joint inflammation between flares, development of tophi (urate crystal deposits under the skin), and progressive joint destruction is not an inevitable destination for people with gout. It is the predictable consequence of years of inadequately treated hyperuricemia. Every year that serum uric acid remains above the saturation threshold, urate crystals continue to accumulate in joints and soft tissues, the NLRP3 inflammasome remains primed for activation, and joint cartilage and bone are silently eroded. At Healthy Feet NYC, our chronic gout treatment program in NYC is designed to interrupt this progression achieving and sustaining target serum uric acid levels, dissolving existing crystal deposits over time, resolving tophi, and restoring joint function that chronic inflammation has compromised.
Chronic gout is distinguished from episodic gout by the pattern of disease: instead of discrete attacks with complete resolution between them, patients with chronic gout experience persistent joint inflammation, chronic pain and stiffness, and the development of tophi chalky white urate deposits visible under the skin, most commonly on the ears, elbows, fingers, and feet. Joint destruction in chronic gout results from both direct urate crystal erosion of cartilage and subchondral bone and from the sustained inflammatory environment that promotes synovial proliferation and enzymatic tissue breakdown. Chronic gout treatment in NYC at Healthy Feet NYC centers on aggressive, sustained urate-lowering therapy reducing serum uric acid to below 5 mg/dL in patients with tophi (a stricter target than the standard 6 mg/dL) to create the concentration gradient that allows existing crystal deposits to slowly dissolve. This process takes months to years of consistent therapy but is achievable in virtually all patients with adherent, well-monitored treatment.

Managing chronic gout effectively requires a fundamentally different mindset than managing acute flares it requires treating the underlying metabolic disorder rather than repeatedly putting out inflammatory fires. Our chronic gout management protocol includes allopurinol or febuxostat titrated systematically to achieve and maintain uric acid targets, with quarterly serum uric acid monitoring and dose adjustment as needed. Prophylactic low-dose colchicine is maintained for a minimum of six months after achieving target uric acid, and often longer in patients with tophi, to prevent the mobilization flares that occur as crystal deposits dissolve and shed shed urate particles into the joint space. For patients with large, functionally impairment-causing tophi, we evaluate the appropriateness of tophi debridement or surgical removal. For patients with renal impairment limiting allopurinol dosing or febuxostat use, we collaborate with nephrology to optimize urate management within the constraints of kidney function. Joint damage already present is assessed and managed with appropriate biomechanical support, orthotics, and activity modification.
