Athlete’s foot (tinea pedis) and toenail fungus (onychomycosis) are both caused by dermatophyte fungi — most commonly Trichophyton rubrum — but they affect different structures of the foot and require different treatments. Athlete’s foot affects the skin, presenting as itching, scaling, redness, and cracking between the toes and on the sole. Toenail fungus affects the nail plate, causing thickening, yellowing, brittleness, and separation from the nail bed. Critically, the two conditions are clinically linked — untreated athlete’s foot is one of the most common pathways for fungal infection to spread into the toenails, and existing toenail fungus acts as a persistent reservoir that reinfects the skin. Athlete’s foot is diagnosed clinically and confirmed with KOH microscopy when needed, and is treated with topical or oral antifungal agents over a course of weeks. Toenail fungus diagnosis requires nail clipping for PAS staining or fungal culture to confirm the organism and guide treatment selection, as non-dermatophyte molds and yeasts require different antifungals than standard dermatophytes. Treatment for onychomycosis is significantly more involved — topical nail lacquers like efinaconazole or tavaborole for mild cases, and oral terbinafine or itraconazole for moderate-to-severe nail involvement, requiring six to twelve weeks of therapy with liver function monitoring in some patients.
