A friction blister forms when repetitive shear forces between skin layers cause separation and accumulation of clear or slightly yellow serous fluid — the body’s protective response to mechanical irritation at the skin surface. A blood blister, by contrast, forms when the same shear or compression forces damage the small blood vessels beneath the skin, causing blood to leak into the blister cavity alongside or instead of serous fluid — producing the characteristic dark red or purple coloration. Blood blisters indicate a deeper level of tissue trauma than friction blisters and should not be drained casually, as the puncture wound in a vascularized blister carries a meaningfully higher infection risk than draining a serous blister. When drainage is necessary due to size, location, or pain, we perform sterile needle aspiration at the blister edge, preserving the roof as a natural wound cover during the healing phase. Blood blisters are managed more conservatively — we typically avoid drainage unless the blister is under significant pressure, causing severe pain, or showing signs of secondary infection — because the blood within creates a richer culture medium for bacterial growth and increases infection risk considerably if the sterile environment is breached. For both types, we apply appropriate wound coverage and offloading padding and provide specific instructions on monitoring for infection signs — increasing redness, warmth, streaking, or systemic symptoms — that warrant immediate return for clinical reassessment.
