Flat Feet Are One of the Most Common Foot Conditions in New York City and One of the Most Undertreated. The Right Specialist Changes That.

Flat feet the partial or complete loss of the medial longitudinal arch affect an estimated 30 percent of the adult population and represent one of the most frequent presentations seen by foot and ankle specialists in New York City. Yet the majority of patients with flat feet either do not know they have the condition or have been told it requires no treatment, despite experiencing pain, fatigue, and mechanical limitation that directly traces to their foot architecture. At Healthy Feet Podiatry, our NYC flat feet doctors evaluate both the structural and functional dimensions of each patient’s flat foot condition and provide treatment precisely calibrated to type, severity, and the degree to which the deformity is affecting daily life.

Flat feet medically termed pes planus describes a foot in which the medial longitudinal arch is reduced or absent on weight-bearing, causing the plantar surface of the foot to contact the ground fully or near-fully. The condition is classified by etiology and flexibility: flexible flat feet, the most common type, exhibit a visible arch when non-weight-bearing that collapses on standing, and are caused by ligamentous laxity, tendon insufficiency, or congenital hypermobility; rigid flat feet exhibit no arch in any position and are typically associated with tarsal coalition, arthritic change, or neurological condition. Adult-acquired flatfoot deformity the progressive loss of arch height in an adult who previously had a normal arch is most commonly caused by posterior tibial tendon dysfunction (PTTD), in which progressive tendon degeneration leads to arch collapse, hindfoot valgus, and forefoot abduction. At Healthy Feet Podiatry, our NYC flat feet specialists classify each patient’s presentation using weight-bearing X-ray, clinical gait assessment, and tendon evaluation before recommending treatment.

Flat feet treatment at Healthy Feet Podiatry is matched precisely to deformity type, severity, and the specific structures involved. Conservative management effective for the large majority of flat feet patients includes custom foot orthotics designed to support the medial arch, control hindfoot valgus, and redistribute plantar load away from overloaded structures; physical therapy targeting posterior tibial tendon strengthening, Achilles tendon stretching, and intrinsic foot muscle activation; supportive footwear with structured arch support and motion control; and anti-inflammatory management for acute tendon or joint pain. For patients with adult-acquired flatfoot deformity driven by posterior tibial tendon dysfunction, early intervention is critical tendon pathology that is caught at Stage I or II responds well to conservative care, while Stage III and IV deformity involving rigid hindfoot and ankle arthritis requires surgical reconstruction. Surgical options range from soft-tissue procedures such as tendon transfer and spring ligament repair, to bony procedures including medializing calcaneal osteotomy, lateral column lengthening, and arthrodesis all performed in accredited outpatient or surgical facilities with individualized recovery planning.
