Adult flat feet encompass several distinct clinical entities that share the appearance of a reduced or absent medial arch but differ significantly in cause, natural history, and treatment requirement. Flexible adult flat feet present since childhood and unchanged in severity typically require management only when symptomatic, with custom orthotics and supportive footwear providing adequate control for the majority of patients. Adult-acquired flatfoot deformity (AAFD) is the most clinically significant category: this is a progressive condition, most commonly caused by posterior tibial tendon dysfunction (PTTD), in which the arch collapses over time due to tendon degeneration and secondary ligamentous failure. AAFD is classified in four stages from isolated tendon inflammation (Stage I) through flexible hindfoot valgus (Stage II) to rigid hindfoot deformity (Stage III) and ankle involvement (Stage IV), and each stage has distinct treatment requirements. Rheumatoid arthritis, Charcot neuroarthropathy, and tarsal coalition represent additional causes of adult flat feet requiring specialized management approaches.

Adult flat feet treatment at Healthy Feet Podiatry is staged to match the deformity’s current classification and trajectory. Stage I PTTD tendon inflammation without deformity responds well to immobilization, physical therapy, and custom orthotics, with the goal of preventing progression to arch collapse. Stage II flexible hindfoot valgus and arch collapse is managed with aggressive orthotic support (a rigid custom orthotic with significant rearfoot posting), Achilles stretching, and activity modification; surgical candidates in Stage II who fail conservative care typically achieve excellent outcomes with soft-tissue reconstruction and calcaneal osteotomy. Stage III and IV rigid deformity and joint arthritis require surgical reconstruction including arthrodesis, with outcomes dependent on maintaining realistic expectations about residual stiffness following joint fusion. For patients with symptomatic flexible adult flat feet without PTTD, conservative management with custom orthotics, strengthening, and footwear guidance typically achieves satisfactory long-term symptom control without surgical intervention.
