Ganglion cyst aspiration is one of the most effective and underutilized treatment options for symptomatic foot cysts and at Healthy Feet NYC, it is a procedure we perform regularly in our Manhattan office with excellent results. Aspiration involves using a needle, under local anesthesia, to drain the thick gelatinous fluid from within the cyst immediately collapsing its volume, relieving the pressure it was exerting on surrounding nerves and structures, and dramatically reducing pain. The procedure takes minutes, requires no incision, involves no stitches, and allows most patients to return to normal activity the same day or the following day. For the right patient with the right cyst in the right location, aspiration is an excellent first-line intervention before considering surgical excision.

Ganglion cyst aspiration in NYC is a minimally invasive office procedure in which a podiatrist inserts a needle into the cyst under local anesthesia and withdraws the thick, viscous synovial fluid filling the cyst cavity. At Healthy Feet NYC, aspiration is performed under ultrasound guidance whenever the cyst’s location, depth, or proximity to neurovascular structures warrants it significantly improving accuracy and reducing the risk of incomplete drainage or inadvertent injury to adjacent tendons or nerves. The procedure typically removes several milliliters of thick, clear-to-yellowish gelatinous fluid. After drainage, the cyst wall collapses, volume decreases markedly, and the pressure-related pain that most patients experience is immediately relieved. Some providers also inject a small volume of corticosteroid into the cyst cavity following aspiration to reduce local inflammation and potentially lower the rate of recurrence, though evidence for this additive step is mixed.

Ganglion cyst aspiration is most appropriate for cysts that are clearly fluid-filled on ultrasound, accessible via needle without risk to adjacent critical structures, causing symptomatic pressure or pain, and in patients who are not yet ready for or not candidates for surgical excision. The main limitation of aspiration is recurrence: studies report recurrence rates ranging from 40 to 70 percent following aspiration of dorsal foot ganglion cysts, compared to 10 to 20 percent following complete surgical excision with stalk removal. This does not make aspiration the wrong choice for many patients, aspiration provides months to years of symptom relief, and repeated aspiration is a reasonable long-term management strategy for patients who wish to avoid surgery or have medical conditions that elevate surgical risk.
