Post-operatively, patients are placed in a non-weight-bearing cast for two to four weeks followed by progressive weight-bearing in a walking boot from weeks four through eight, with formal physical therapy beginning at six weeks targeting first MTP range of motion, intrinsic strength, and push-off mechanics. Return to sport follows objective functional criteria assessment at four to six months post-operatively not a fixed calendar date.
