The posterior tibial tendon (PTT) is the primary dynamic stabilizer of the medial longitudinal arch running from the tibialis posterior muscle behind the medial malleolus to insertions on the navicular, cuneiforms, and metatarsal bases on the bottom of the foot. Posterior tibial tendon dysfunction (PTTD) is the most common cause of adult-acquired flatfoot deformity, progressing through four clinical stages: Stage I involves tendinitis and early tendon degeneration without deformity; Stage II involves partial tearing with flexible flatfoot deformity; Stage III involves complete tendon incompetence with rigid flatfoot; and Stage IV adds valgus ankle joint involvement. The tragedy of PTTD is that Stage I and early Stage II are highly amenable to conservative treatment and full functional recovery but most patients present at Stage II or III because the gradual onset of medial ankle pain and arch flattening is dismissed for months. At Healthy Feet Podiatry, our NYC PTT specialists use diagnostic ultrasound and weight-bearing foot and ankle X-ray to stage every patient accurately and initiate the appropriate intervention before irreversible deformity develops.
At Healthy Feet Podiatry, PTT tear treatment is organized by deformity stage and tendon integrity. Stage I PTTD is managed with immobilization in a walking boot during the acute inflammatory phase, custom UCBL orthotics with medial posting to support the arch and offload the tendon, and a structured tibialis posterior strengthening program. Stage II PTTD with partial tearing and flexible flatfoot is treated with extended orthotic management, physical therapy, and PRP injection for documented tendon degeneration with surgical referral for patients who fail six months of structured conservative treatment. Stage III rigid flatfoot deformity and Stage IV cases require surgical reconstruction including procedures such as medial calcaneal osteotomy, flexor digitorum longus tendon transfer, and lateral column lengthening which we coordinate with our affiliated reconstructive foot and ankle surgical partners. Post-surgical rehabilitation is managed by our NYC team throughout the full recovery pathway.
