Discolored Toenail — Fungus or Trauma? NYC’s Podiatrists Tell You Exactly What You’re Dealing With.

Toenail discoloration is one of the most common reasons patients present to Healthy Feet Podiatry’s NYC offices — and one of the most frequently self-misdiagnosed conditions in podiatric dermatology. Onychomycosis (toenail fungus) and traumatic nail dystrophy produce strikingly similar clinical appearances — both can cause yellow-brown discoloration, nail thickening, subungual debris, and nail plate irregularity but they have entirely different causes and require entirely different treatments. Traumatic nail discoloration results from repetitive microtrauma most commonly from shoes that are too short or narrow, causing the nail to strike the toe box during walking and running which produces subungual hemorrhage, nail bed fibrosis, and secondary nail plate thickening that mimics fungal infection on visual inspection. Key distinguishing features include the bilateral symmetry of traumatic nail changes across the same toe on both feet, the absence of fungal hyphae on KOH microscopy, and a clear history of ill-fitting footwear or high-impact activity. At Healthy Feet Podiatry, we confirm or exclude onychomycosis with laboratory testing KOH microscopy and PAS-stained nail clipping before prescribing any antifungal treatment, because months of antifungal therapy directed at a traumatic nail will produce no improvement and expose the patient to unnecessary cost and medication risk.

At Healthy Feet Podiatry, confirmed onychomycosis is treated with the appropriate antifungal regimen based on severity and organism topical lacquers for mild presentations, oral terbinafine or itraconazole for moderate-to-severe infections. Confirmed traumatic nail dystrophy, by contrast, is managed by eliminating the causative mechanical stress footwear assessment and correction to ensure adequate toe box length and width, nail plate trimming to prevent further impaction, and protective nail padding for patients who cannot immediately change footwear. In cases where traumatic nail change has produced significant nail thickening causing shoe pressure pain, we perform professional nail debridement to restore nail comfort while the underlying nail bed heals over three to six months. For patients where both conditions coexist traumatic nail thinning and widening that has created a fungal entry point we treat both the mechanical cause and the infection simultaneously with a coordinated management plan.
